Ductal Carcinoma in Situ

This is potentially a step before cancer. It is thought that not all DCIS will progress to cancer, but currently we are not certain which types of DCIS will or won’t. That is why currently the recommendation is typically to excise DCIS.

The surgical options for DCIS mimics invasive cancer – that is lumpectomy or mastectomy. Lumpectomy though can only be offered when considering the size of DCIS relative to the woman’s breast. When a patient chooses a lumpectomy they must be aware that they made need radiotherapy afterwards. Dr Ofri is a strong advocate for DCISionRT, a personalised test that can be performed, to determine whether a patient would benefit or not from radiotherapy.

When a patient requires, or prefers, a mastectomy (removal of all breast tissue) breast reconstruction should always be discussed. It is important to note, that when having a mastectomy, you will also need an evaluation of the lymph nodes in the armpit, much like in cancer surgery.

Invasive Carcinoma

it is important to know that breast cancer has one of the best cancer prognoses (outcomes). Given how common it is, we have learnt so much and are constantly further evaluating and researching better ways to treat it.

Typically breast cancer is just sporadic, it just happens. However there are genetic causes of breast cancer which could be inherited by family members. Dr Ofri has a strong interest in the genetics of breast cancer and will discuss the need of any further testing as required.

When you have a breast cancer, your surgery not only includes your breast, but also the lymph nodes in your arm pit, to help stage and ensure it hasn’t spread. Breast surgery options are dependant on many factors and surgery, though pivotal in breast cancer management, isn’t the only aspect.
